Application for Service (PDF)
This packet has general information about the PUD and mandatory forms needed to set up a new account for water service.

Authorization to Bill Tenant Form (PDF)
This form must be completed by the Property Owner or Property Manager and returned to Thurston PUD, before utility bills can be transferred into a tenant’s name. Please note: a Property Manager must already be authorized on an owner account before our staff can accept an Authorization to Bill Tenant Form from the Property Manager - see the Authorization for Property Manager Form below.

Authorization for Property Manager (PDF)
This form must be completed and returned to Thurston PUD, to authorize a Property Manager on an owner account.

Cross-Connection Control Form (PDF)
This form is required to be completed at least every five (5) years or when changes to customer plumbing has been completed, as part of the PUD’s Cross-Connection Control Plan (Resolution 05-15), developed in compliance with Washington Administrative Code (WAC) 246-290-490.

Backflow Tester List (PDF)
A curated directory of certified Backflow Assembly Testers qualified to inspect, test, and repair backflow prevention assemblies. Customers with backflow devices must complete testing annually.

Claim For Damages Form (PDF)
This form must be used to file a claim with Thurston PUD when an incident occurs, and an involved party believes the District may be responsible for the damage costs.

Direct Debit Application (PDF)
Complete and return this form to set up Auto Pay using your checking or savings account. If you’d like to set up Auto Pay from a Visa or Mastercard instead, please visit www.thurston.merchanttransact.com/Login to set up an account online.

Request for Public Records
Click the link above to learn about how to a request for public records. Thurston PUD public records requests are processed in accordance with Chapter 42.56 RCW, the Public Records Act.

Commercial Purpose Declaration - Public Records Requests (PDF)
If your public records request includes a request to access one or more lists of individuals, you must use this form to declare that you will not use the list for a commercial purpose.

Project Help Application For Assistance (PDF)
This application can be completed by Thurston PUD low-income customers to be considered for financial assistance to be applied to their customer account.

Customer Information Update Form (PDF)
Complete and return this form to update your contact information on your existing Thurston PUD account. Please note, this form cannot be used to change the account name.

Water Availability Forms (WAL)
Thurston PUD - Water and Fire Flow Availability Application  (PDF)
This application is required if you are requesting Water Availability documentation for a parcel in Thurston PUD’s service area and if you are applying for a building permit, starting a new plat development project, or need a Certificate of Water Availability.
